AECOPD患者早期介入中医肺康复的随机对照研究

Randomized Controlled Study of Early Intervention with TCM Pulmonary Rehabilitation in AECOPD Patients

目的观察以肺部感染控制窗(PIC窗)为切入点对慢性阻塞性肺疾病急性加重期(AECOPD)患者进行为期6个月"六字诀"中医肺康复后的临床疗效评价。方法将AECOPD患者随机分为中医康复组和对照组,以PIC窗开启为切入点,中医康复组予以练习传统中医功法"六字诀"联合西医常规治疗,对照组予以西医常规治疗,总时间为6个月,观察两组治疗前后辅助呼吸肌动用评分、呼吸困难评分(MRC评分)、6min步行距离(6MWT),并记录其呼吸频率、血压及指末氧饱和度的前后变化。结果两组治疗前后辅助呼吸肌动用评分、MRC评分、6MWT均有显著差异,中医康复组明显优于对照组;两组呼吸、心率、血压及指末氧饱和度治疗前后均无明显差异。结论对AECOPD患者以PIC窗为切入点行中医肺康复治疗,可以缓解呼吸肌疲劳,改善呼吸困难,提高活动能力,具有一定的可行性。

Objective：To study the clinical effect of TCM pulmonary rehabilitation with ＂Six Letters Formula＂ for 6 months in the AECOPD patients in the starting point of pulmonary infection control window.Methods： 80 patients were randomly divided into the traditional rehabilitation group and the control group.As the starting point of pulmonary infection control window,the control group was given the western routine therapies.Meanwhile,the traditional rehabilitation group was used the ＂ Six Letters Formula＂ of TCM exercises besides those treatment in the other group.After 6 months,some indices were observed before and after treatment including the change of dyspnea score,the 6 minutes walking distance,the accessory muscle use score,the respiratory frequency,the blood pressure and the finger trip blood oxygen saturation.Results： These indices in the traditional rehabilitation group were obviously superior to those of the control group such as the accessory muscle use score,the dyspnea score and the 6 minutes walking distance.However,there were no statistical differences between 2 groups in the respiratory frequency,the heart rate,the blood pressure and the finger trip blood oxygen saturation.Conclusion： It,that is feasible,can relieve respiratory muscle fatigue,improve shortness of breath and increase activity that the TCM pulmonary rehabilitation to AECOPD patients as the starting point of pulmonary infection control window.
