广东地区性病门诊男性患者人乳头瘤病毒感染及其基因分型研究 被引量:10

摘要 目的了解广东地区男性性病患者人乳头瘤病毒(HPV)感染及其基因型。方法用导流反向斑点杂交法对357例性病门诊男性患者进行HPV检测与基因分型,评价HPV及其基因型与临床症状及疾病类型的相关性。结果 357位受检者中,HPV总阳性率为43.7%(156/357),包皮、龟头和阴茎标本的HPV阳性率为44.9%(136/303)。赘生物、丘疹、红斑和无症状样本的HPV感染率分别为81.0%(94/116)、53.1%(17/32)、40.0%(4/10)和20.5%(38/185)。赘生物的HPV检出率与丘疹、红斑和无症状样本的HPV检出率差异有统计学意义(P<0.05)。基因型分析发现,5例鲍温样的丘疹病患者均有高危型HPV感染,而78例尖锐湿疣患者中低危型HPV感染病例占93.6%(73/78)。21种HPV基因型均被检出,以6(22.1%)、11(21.7%)、16(11.2%)、52(5.6%)、18(4.8%)和58(4.0%)型感染最常见。结论 HPV无症状感染在性病门诊男性就诊者中较常见,HPV基因型分布与疾病类型密切相关。 Objective To study the correlation between infection of human papillomavirus(HPV) and distribution of HPV genotype in male patients.Methods The swab samples of 357 male patients aged from 16 to 68 years old were collected.Twenty-one genotypes of HPV were detected by PCR and reverse line blot hybridisation.The correlation between HPV genotypes and clinical symptoms was analyzed.Results The total positive rate of HPV infection was 43.7%(156/357) in the tested male patients.Among them,the positive rate of HPV infection in the samples from glans/corona and penis was 44.9%(136/303).The prevalence of excrescence,papule,erythema and asymptomatic infection was 81.0%,53.1%,40.0% and 20.5% respectively,and significant differences of HPV infection among the different symptom groups(P〈0.05) were found.The five patients with bowenoid papulosis were all infected by high risk HPV.The positive rate of HPV in the patients with condyloma acuminate,who were genotype of moderate or low risk HPV,was 93.6%(73/78).All of 21 HPV genotypes were detectable.The genotypes with high prevalence successively were HPV6(22.1%),HPV11(21.7%),HPV16(11.2%),HPV52(5.6%),HPV18(4.8%) and HPV58(4.0%).Conclusion The asymptomatic infection of HPV was common in the men attending venereal disease clinics.HPV genotypes may associate with the symptoms of HPV infection.
