2004-2008年武鸣县初治涂阳肺结核病人治疗失败情况分析

Analysis of causes of failure in treatment of smear positive pulmonary tuberculosis patients in Wuming County in 2004～2008,Guangxi

目的分析广西武鸣县2004～2008年间初治涂阳肺结核病人治疗失败的情况,为今后制定相应的防治对策提供科学依据。方法收集2004-2008年武鸣县结核病控制项目肺结核病人登记治疗资料并进行统计分析。结果2004～2008年间初治涂阳肺结核病人治疗失败率为2.9%,男性初治失败率高于女性,不规律治疗病人治疗失败率为7.9%明显高于规律治疗病人的2.4%(χ2=6.65,P<0.05),二月末痰涂片阴性病人治疗失败率,远低于二月末痰涂片阳性病人(χ2=56.9,P<0.001)。结论加强初治病人宣教,提高病人治疗依从性,对疗效不满意或二月末痰检仍阳性的初治病人,及时做结核菌培养及药敏试验,针对个体调整治疗方案,提高病人治愈率。

Objective To analyse the causes and status of failure in treatment of smear positive pulmonary tuberculosis patients in Wuming county from 2004 to 2008 and provide scientific evidence for formulating effective treatment measures.Methods The registered data concerning treatment of pulmonary tuberculosis patients in Wuming County during the control program from 2004 to 2008 were collected and statistically analazed using SAS 9.0 software.Results The failing rate in treatments of smear positive pulmonary tuberculosis patiens from 2004 to 2008 was 2.9%.The failing rate in initial treatment of male patients was higher than that of female patients.Failing rate of irregular treatment of pulmonary tuberculosis was significantly higher comopared to that regularly treated patients（χ2=6.65,P0.05）.The treatment failure rate of smear negative pulmonary tuberculosis patients at end of two months was far lower than that of smear positive ones（X2=56.9,P0.001）.Conclusions Effective measures including strengthening health education,enhancement of pateints＇ compliance,regular treatment and individualized treatment be carried to improve the cure rate.
