银屑病关节炎的X线诊断并文献复习(附1例报告)

X-RAY DIAGNOSIS OF PSORIATIC ARTHRITIS:LITERATURE REVIEW AND REPORT OF ONE CASE

目的探讨X线平片在银屑病关节炎诊断中的价值和意义。方法对皮肤病学、风湿病学、放射诊断学等领域中的有关文献和专著中有关本病的论述进行复习,并对具体病例诊断进行分析。结果 X线平片在银屑病关节炎的诊断中占有重要地位,但在与类风湿关节炎、强直性脊柱炎的鉴别诊断中存在困难。它们之间具有不同之处的同时又具有很多相似之处,银屑病关节炎主要表现为一种不典型性的特点。结论银屑病关节炎的确切定义应为伴发银屑病的不典型关节炎。由于本病的疼痛严重影响病人生活质量,到晚期可以致残,故本病应引起医学界的重点关注。

Objective To assess the value and significance of radiography in the diagnosis of psoriatic arthritis（PA）.Methods Literature and monograph concerning PA in the domains of dermatology,rheumatology,and radiology were reviewed and specific cases were analyzed.Results X-ray films showed an important place in the identification of PA,whereas there was still a difficulty in differential diagnosis of PA from rheumatoid arthritis and ankylosing spondylitis.There were differences among them,and,at the same time,there were good many similarities.The main manifestation of PA was the feature of untypical.Conclusion The precise definition of psoriatic arthritis should be atypical arthritis accompanied by psoriasis.Because the pain caused by this disease severely affects patients＇ quality of life,and results in disability at advanced stage,a good deal of attention should be paid to this condition.
