摘要
目的研究间接免疫荧光法(IIF)测定尖锐湿疣疣体表面细胞中人乳头瘤病毒的敏感性和特异性,探讨间接免疫荧光法在尖锐湿疣早期诊断中应用的可行性。方法收集尖锐湿疣标本81例,同时设立对照组52例,分别用刮片法(刮取疣体表面细胞法)及涂片法(切除疣体涂片)取材,用间接免疫荧光法检测。并与组织病理诊断和醋酸白试验检查结果比较。结果81例尖锐湿疣标本中,刮片法42例阳性,敏感性和特异性分别为51.8%和100%;涂片法64例阳性,敏感性和特异性分别为79.0%和100%;对照组均阴性。刮片法的敏感性低于涂片法(P<0.01)。结论用表面刮取脱落细胞法取材,用间接免疫荧光法检测HPV的敏感性不高,但特异性较强,可以作为诊断早期尖锐湿疣的辅助手段。
Objective To assess sensitivity and specificity of indirect immunofluorescence(IIF) technique in detection of human papillomavirus(HPV) in the surface cells of condyloma accuminatum(CA) and discuss the feasibility of IIF in early diagnosis of CA.Methods 81 cases of CA and 52 cases of non-CA served as controls were included.Cells scraping from the surface of the CA were examined as the f irst method,smear samples from excised tissue of CA were analyzed as the second method.The specimens were examined by means of IIF,acetowhitening and biopsy separately.Results Specific fluorescence was found in 42 of the 81 cases,and in none of the 52 control subjects by the fi rst method.The sensitivity and specifi city of the fi rst method was 51.8% and 100% respectively.Specific fluorescence was found in 64 of the 81 cases,and in none of the 52 control subjects for the second method.The sensitivity and specifi city of the second method was 79.0% and 100% respectively.The sensitivity of the first method is significant lower than the second one(P<0.01).Conclusion Using IIF to detect HPV in Scraping cells from the surface of the CA is a method with moderate sensitivity and good specifi city.It may become a useful method in early diagnosis of CA.It may also increase the probability of early detection of HPV in CA.
