重症胰腺炎并发胰腺假性囊肿的治疗

The treatments of pancreatic pseudocysts

目的:探讨重症胰腺炎并发胰腺假性囊肿的治疗方法。方法:对46例胰腺假性囊肿患者的临床资料进行回顾性分析。结果:非手术治疗9例,其中4例在B超引导下行囊肿穿刺抽液;常规外科手术治疗19例,其中囊肿胃吻合术2例,囊肿空肠Roux-en-Y吻合术14例,囊肿单纯切除术3例;内镜手术治疗18例,其中在X线引导下的囊肿穿刺术10例,在超声内镜的引导下囊肿穿刺术8例。结论:胰腺假性囊肿可出现多种并发症,是急性重症胰腺炎后治疗的难题。非手术治疗适用于囊肿形成的早期,囊肿空肠Roux-en-y吻合术是一种简单易行而疗效可靠的方法,而内镜下手术则是今后手术治疗的发展方向。

[Objective]To determine the methods and effective treatments of pancreatic pseudocysts.[Method]The clinical data of 46 patients with pancreatic pseudocysts were retrospectively analyzed.[Result]Among these patients,nonoperative therapy: 9 cases;Operation:19 cases;X ray guided Aspiration:10 cases;Endiscopic ultrasonography guided aspiration:8 cases;[Conclusion] Pancreatic pseudocysts tend to multiple complications.Nonoperative therapy is useful at early stage.Aspiration is the future tendency.
