凝聚胺法和抗人球蛋白法在交叉配血试验中的应用

Application of polybrene test and antiglobulin test in blood cross-matching

目的:对凝聚胺法和抗人球蛋白法在交叉配血试验中结果不符进行分析,提出此情况下准确发布实验结果和及时解决问题的方案。方法:收集了8例临床交叉配血结果异常的病例,ABO以外抗体筛查和自身对照实验采用抗人球蛋白法,交叉配血试验采用盐水法、聚凝胺法和抗人球蛋白法。结果:ABO以外抗体筛查阳性,自身对照实验为阴性,8例病例均出现盐水法和聚凝胺法的主侧无凝集,而抗人球蛋白法的主侧凝集。结论:凝聚胺法在筛查IgG类抗体的灵敏度不如抗人球蛋白法,采用凝聚胺法和抗人球蛋白法进行交叉配血试验,遇到异常的实验结果时,应以抗人球蛋白法为准。

Objective:To analyze the incompatibility of the blood cross-matching results using polybrene test and antiglobulin test and propose the instant solution for accurate declaring test results under such circumstance.Methods:8 cases with abnormal clinical blood cross-matching result were detected using antiglobulin test for irregular antibody screening and own control experiment and using Sodium Chloride test,polybrene test and antiglobulin test for blood cross-matching.Results:Irregular antibody screening test was positive,own control experiment was negative.All of the 8 cases showed major cross-matching test non-agglutination using Sodium Chloride test and polybrene test,yet using antiglobulin test showed major cross-matching test agglutination.Conclusion:Polybrene test is not as sensitive as antiglobulin test in screening IgG antibody.If inconsistent test results appear while using polybrene test and antiglobulin test for blood cross-matching,we should prioritize antiglobulin test.
