摘要
目的:观察手法治疗寰枢椎半脱位并发眩晕的疗效并探讨其治疗机理。方法:将72例寰枢椎半脱位并发眩晕症的患者,随机分为两组,治疗组36例运用手法治疗。对照组36例运用牵引治疗。结果:治疗组治愈9例,显效18例,好转7例,无效2例,总有效率达94.44%,愈显率为75%;对照组治愈3例,显效8例,好转14例,无效11例,总有效率为69.44%,愈显率为30.53%,两组疗效比较,经统计学Ridit处理,有显著性差异(m=3.6459,P<0.01)。说明治疗组疗效优于对照组。结论:手法治疗有较好的疗效,其原理为手法治疗可以通过纠正寰枢椎的异常解剖位移及生物力学关系,减轻或消除局部的创伤性炎症反应,恢复关节之内外平衡,消除压迫作用,使交感神经紧张性减低,反射性引起血管扩张,恢复血管壁弹性功能,使组织代谢旺盛,血流增加,脑组织供血改善,有利于症状的缓解,从而达到治疗的目的。
Objective:To observe the therapeutic results of atlantoaxial subluxation concomitant with vertigo treated with manipulations and to discuss on its mechanism. Methods:72 patients of atlantoaxial subluxation concomitant with vertigo were randomly and equally divided into two groups, the treatment group treated with manipulation and the control group treated with traction therapy. Results:9 cases were cured, 18 markly improved, 7 improved and 2 uncured in the treatment group with a total effective rate of 94.44% and improving rate of 75%, while in the control group 3 were cured, 8 markly improved, 14 improved and 11 uncured with a total effective rate of 69.44% and improving rate of 30.53%. Statistics shows that there is obvious difference in therapeutic results between two groups (m=3.6459, P<0.01). It indicates that the therapeutic results of the treatment group are superior to that of the control group. Conclusion:Manipulation has good results on treating atlantoaxial subluxation concomitant with vertigo. The mechanism is as follows: with manipulation treatment, the deviated vertebrae was realigned to its normal anatomy position, the balance of the joint in and out was recovered, the narrowing of the space was relieved to reduce the tension of the sympathetic nerve and to dilate the reflex vessel, the elasticity of the vascular wall was regained with high metabolism of tissue, promoting blood circulation and improving blood supply for the brain, which benefits for relieving the symptoms.
