腓肠神经营养血管岛状皮瓣的临床应用

THE CLINICAL APPLICATION OF ISLAND FLAP COMBINED WITH THE NUTRIENT VESSELS OF SURAL NERVE

目的 总结腓肠神经营养血管岛状皮瓣的临床应用效果。方法 1998年开始应用腓肠神经营养血管岛状皮瓣修复膝、小腿、踝及足部皮肤软组织缺损 18例 ,其中顺行蒂 3例 ,逆行蒂 15例。皮瓣面积 5cm× 7cm～ 8cm× 16cm。血管蒂长 9～ 16cm ,蒂宽 2～ 3cm。结果 术后 17例皮瓣完全成活 ,1例皮瓣尖端部分皮肤坏死 ,经换药愈合。经 3个月～ 1年随访 ,皮肤质地及外形较好。结论 该皮瓣血供可靠 ,不牺牲肢体主干动脉 ,操作简单 ,是修复膝、小腿、踝及足部皮肤软组织缺损较好方法。

Objective To assess the clinical application efficacy of the island flap combined with the nutrient vessels of sural nerve. Methods Since 1998,the island flaps combined with the nutrient vessels of sural nerve were performed to repair the soft tissues defects in the knee, leg, ankle and foot in 18 cases. 3 cases were proximal base and 15 cases were distal base. The sizes of flap were about 5 cm×7 cm to 8 cm×16 cm. The lengths of the pedicel of the blood were 9 cm to 16 cm and the widths were 2 cm to 3 cm. Results Seventeen flaps survived completely.one flap was partially necrosed in the tip and the wound healed after changing dressing. Followed up 3 to 12 months, the flaps showed fine texture and appearance. Conclusion This kind of flap has advantages as follows: blood supply reliability, avoidance of the damage of major arteries, and procedure simplification. This is a good method to repair the soft tissues defects in position of the knee, leg, ankle and foot.
