摘要
目的 报告完全内窥镜下房室间隔缺损修补术 1 2 5例的结果。方法 2 0 0 0年 6月～ 2 0 0 2年 8月 ,于右侧胸壁打孔内窥镜下房室间隔缺损修补手术 1 2 5例。直接缝合 1 1 3例 ,涤纶补片修补 1 2例 ,同时行三尖瓣成形 2 2例 ,右室流出道狭窄解除 2例。结果 术中扩大切口 3例 ,室缺残余漏 1例 ,二次开胸止血 2例 ,手术时间 2 .1～ 5 .6h ,平均 (3 .1± 0 .6)h。体外循环时间 38～ 1 68min ,平均 (78± 2 5)min ;升主动脉阻闭时间 9～ 56min ,平均 (2 5± 8)min。术后患者恢复顺利 ,治疗效果满意。结论 完全内窥镜下的房室间隔缺损修补术是可行的、安全的。
Objective\ To summarize the experiences of totally thoracoscopic closure of heart septal defect. Methods\ 125 patients (61 male and 64 female) underwent operation for closure of a heart septal defect. The operations were performed through three right anterior holes under thoracoscopy. 113 patients underwent direct suture colsure, 12 patients underwent dacron patch, 22 patients underwent tricuspid annuloplasty for tricuspid regurgitation. Results\ All patients survived after surgery. The durations of extracorporeal circulation and cross-clamped respectly were 38～168 minutes, mean(78±25)minutes and 9～56 minutes, mean(25±8) minutes. Postoperative courses were successful and satisfactory. Conclusion\ Totally thoracoscopic closure of heart septal defect is technically feasible and can be used with excellent results.
