关节镜治疗胫骨髁间前棘骨折

THE TREATMENT OF THE FRACTURES OF TIBIAL INTERCONDYLER EMINENCE UNDER ARTHROSCOPE

目的 探讨关节镜在胫骨髁间前棘骨折治疗中的应用。方法 2 0 0 1年 9月～ 2 0 0 3年 3月共治疗 16例胫骨髁间前棘骨折的患者。在关节镜监视下行骨折两断端清创、复位 ,然后由胫骨粗隆两侧钻孔 ,细钢丝横穿前交叉韧带基底并经骨床上的钻孔及骨隧道引出关节外打结、固定。结果 共有 14例获得随访 ,随访时间 2～ 12个月 ,平均 7.5个月。膝关节活动度正常 12例 ,活动受限 2例 ,但均超过 10 0°。 X线片示均为解剖复位。结论 关节镜手术治疗胫骨髁间前棘骨折可最大程度地减少创伤 ,手术简单易行 ,并发症少 。

Objective\ To study the application value of arthroscope for the treatment of the fractures of tibial intercondyler eminence. Methods\ 16 cases of avulsion fracture of tibial intercondylar eminence were treated under arthroscope from September 2001 to March 2003. 7 cases were Ⅱ degree fracture and 9 cases were Ⅲ degree fracture. Arthroscopic routine incisions were performed. After debridement and reduction, thin wire was put into the joint and passed through the base of the fracture pieces, two ends of the wire were pulled out of the joint and tightened. Results\ Fourteen patients were available for follow-up 2～12 months( mean 7.5 months ). Twelve patients gained normal range of motion of knee joint, two patients had limited flexion but exceeded 100 degrees. Bone union and anatomical reduction were obtained in all cases from X-ray plain. Conclusion\ The treatment of the avulsion fracture of tibial intercondyler eminence with arthroscope is simple and effective method and gives less trauma. This method should be used extensively in clinic.
