摘要
目的探讨原因不明复发性流产的发病机制。 方法观察 5批自然流产小鼠模型CBA/J×DBA/ 2(实验组 )的胚胎丢失率 ,并采用ELISA方法检测CBA/J小鼠外周血Th1型、Th2型细胞因子变化。 结果 5批实验中 ,实验组的胚胎丢失率为 2 7.2 7%~ 32 .2 0 % ,均明显高于各自的对照组 (P<0 .0 0 1 ) ,而批间胚胎丢失率差异无显著性 (P>0 .0 5 ) ;其总的胚胎丢失率 (2 9.0 2 % )也显著高于对照组 (5 .0 2 % ,P<0 .0 0 1 ) ;细胞因子检测结果显示流产组外周血Th1型细胞因子IL - 2、TNF -α浓度分别为 36 .3± 1 5 .1 pg/ml、1 4 0 4.9± 1 4 4.9pg/ml,显著高于对照组的9.3± 1 .9pg/ml、6 .9± 1 .6pg/ml,而Th2型细胞因子IL - 4、IL - 1 0浓度分别为 5 8.8± 33.1 pg/ml、32 .0± 1 9.1 pg/ml,显著低于对照组的 1 2 2 6 .5± 1 4 9.1 pg/ml、1 32 .8± 5 8.6pg/ml(P<0 .0 1 ,P<0 .0 0 1 )。 结论CBA/J×DBA/ 2胚胎丢失率显著增加 ,较为稳定 ,是研究复发性流产的理想模型 ;Th细胞在妊娠免疫耐受的形成机制中起重要作用 ;Th1、Th2型细胞因子异常及平衡失调与流产的发生有关。
Objective To investigate the pathogenesis of unexplained recurrent spontaneous abortion(URSA). Methods We observed the embryo loss rate (ELR)in the CBA/J ×DBA/2 murine spontaneous abortion model in 5 separate batches(experimental group) as compared with CBA/J ×BALB/c (non-abortion prone, control group)and determined the concentration of type Th1、Th2 cytokine from female CBA/J. Results In 5 separate batches, the ELR of CBA/J ×DBA/2 (27.27%~32.20%)was increased significantly as compared with the control group respectively, there was no difference among these separate batches; the total ELR (29.02%)was much higher than that of the control group too (5.02%,P<0.001). In experimental group, the concentration of Th1 cytokine IL-2(36.3±15.1pg/ml)and TNF-α(1404.9±144.9pg/ml)were increased significantly as compared with those of control group(IL-2, 9.3±1.9pg/ml;TNF-α,6.9±1.6pg/ml), however, the concentration of Th2 cytokine IL-4( 58.8 ±33.1pg/ml) and IL-10(32.0±19.1pg/ml)was decreased significantly as compared with that of control group(IL-4, 1226.5±149.1pg/ml;IL-10,132.8± 58.6 pg/ml),(P<0.01,P<0.001) Conclusion The results suggested that the ELR of CBA/J×DBA/2 was increased significantly but rather stable. CBA/J×DBA/2 mating could be taken as a good model of URSA. Th cell plays an important role in immunological tolerance during pregnany, abnormal concentration and imbalance of Th1/Th2 is correlated to URSA.
