维拉帕米对卵巢癌患者化疗增敏作用的评价

The evaluation of curative effect and prognosis of verapamil combined with chemotherapy in the treatment of ovarian carcinoma

目的 探讨维拉帕米(VPL)参与下的化疗对卵巢癌细胞多药耐药基因及凋亡因子表达的作用以及对预后的影响。方法 70例首次卵巢癌手术及19例复发患者被随机分为VPL组和非-VPL组,从上述患者化疗前后的腹水中富集癌细胞,应用3种荧光单克隆抗体P_(170)、CD_(95)及Apo_(2.7)标记癌细胞,经流式细胞仪定量检测;两组均进行了随访。结果 应用VPL组化疗后癌细胞CD_(95)及Apo_(2.7)表达含量均高于非-VPL组;VPL对癌细胞P_(170)表达无明显影响;复发组癌细胞P_(170)表达显著高于初治手术组;化疗可使外周血淋巴细胞CD_(95)及Apo_(2.7)表达增加;初步随访结果证实VPL组存活时间显著长于非-VPL组。结论 VPL对加强化疗疗效,特别是对耐药性患者具有重要的临床应用价值。

Objective To investigate the curative effect and prognosis of calcium antagonist-verapamil (VPL) combined with chemotherapy in the treatment of advanced ovarian carcinoma by studying the cellular apoptosis and Mdr-1 expression (P170) before and after chemotherapy. Methods Seventy primary and nineteen recurrent patients with ascites suffered from advanced ovarian carcinoma were divided into VPL group and non-VPL group.Trie samples of ascitic cancer cells (OCC) were collected,immunologically marked by three kinds of fluorescent monoclonal antibodies-PI70,CD95 and Apo2.7 and then examined by flow cytometry. All the patients were followed up. Results The CD95 and Apo2.7 expression contents of OCC were statistically more improved in VPL group than non-VPL one ( P < 0.05). Trie Mdr-1 expression contents (P170) of OCC was not affected by VPL. The PI70 contents were statistically more improved in recurrent group than primary one ( P < 0.05). The CD95 and Apo2.7 contents of peripheral lymphocytes were also improved in VPL group. The survival was obviously longer in VPL group than non-VPL one. Conclusion This suggested that VPL combined with chemotherapy could obviously improve the curative effect and prognosis in ovarian carcinoma and showed important clinical significance,especially in those drug-resistant patients.
