氯氮平治疗精神分裂症剂量与血浓的相关性及性别差异

Relationship between dosage and serum concentration of clozapine in schizophrenia and the gender differences

目的:了解氯氮平治疗精神分裂症用药剂量与血浓的相关性及其性别差异.方法:采用pearson相关分析考察氯氮平用药剂量与血清氯氮平浓度的相关性.比较不同性别患者的氯氮平剂量、血浓及浓度剂量比的差异.结果:氯氮平血清浓度与剂量呈正相关(r=0.33,n=3 172,P<0.01),浓度剂量比变异系数达77%;女性患者的浓度剂量比显著高于男性(P<0.01),女性用药剂量低于男性,但血浓显著高于男性(P<0.01);使用常规剂量的患者中约半数血浓低于300ng/ml.结论:氯氮平血清浓度个体差异大,女性患者血浓高于男性.

AIM: To study the relationship between dosage and serum concentration of clozapine in schizophrenia and the gender differences. METHODS :Pearson relationship anal-ysis was used to study the relationship between dosage and serum concentration of clozzap-ine, the gender differences of clozapine dosage,serum concentration and concentration/dosage ratio were compared. RESULTS : A positive relationship between serum level and dosage of clozapine was found(r= 0. 33, n = 3 712 ,P<0. 01). The CV% of concentration/dosage ratio was as high as 77%. The concentration/dosage ratio of the femal patients was significantly higher than that of the male patienst (P<0. 01). The dosage of clozapine of the femal patients was lower, but the serum concentration was significantly higher than that of the male pa-tients (P<0. 01). Among the patients treated with routine dosage of clozapine, there were nearly half ones with the serum concentration of clozapine less than 300 ng/ml. CONCLU-SION: The individal variance of serum concentration of clozapine is significant. Femal patients' serum concenration of clozapine is higher than that of the male.
