老年病人单肺通气期间SpO_2与SaO_2以及P_(ET)CO_2与PaCO_2的相关性分析

摘要 为探讨老年病人全麻手术单肺通气期间SpO_2与SaO_2以及P_(ET)CO_2与PaCO_2的相关性,选择胸外科择期手术病人16例,男10例,女6例,年龄62.3±12.1岁,单肺通气时间65~120 min。分别于双肺通气10 min及单肺通气 15、30、45及 60 min记录SpO_2和 P_(ET)CO_2值,同时于足背动脉采血行血气分析。结果表明双肺通气时,SaO_2始终为99.9%,SpO_297%~100%;单肺通气各时点SpO_2与SaO_2均呈高度正相关(P<0.O1);双肺通气及单肺通气45 min内,P_(ET)CO_2与PaCO_2均呈高度正相关,但单肺通气 60 min时,二者相关性明显下降(P>0.05)。结论:老年人单肺通气期间SpO_2和 P_(ET)CO_2作为无创监测手段可较好地反映 SaO_2及 PaCO_2的水平,但长时间(≥60 min)应用P_(ET)CO_2,则应结合临床综合评价其意义。
