摘要
目的为了继续探讨经导管关闭继发孔房间隔缺损(ASD)的临床应用。方法继用Sideris的纽扣式补片关闭ASD后,我们又尝试采用CardioSEAL装置关闭ASD4例。此补片为蚌状夹式双伞型闭合器装置,有12、17、23、28、33、40等型号。根据球囊测量ASD最大伸展直径选择补片,其比例为1.7~2.2:1。结果4例均为一次关闭成功,其中一例为两个房缺孔。目前,所有病例仍在追踪随访中,暂未发现有补片移位,支架断裂,心脏其它结构受影响或残余分流等并发症。结论本方法操作较易掌握,补片不易移位。但由于仅适用小于20mm的ASD病例,且价格较贵。所以,手术适应证的选择要掌握好,避兔增加病人的痛苦。此方法仍在临床试用阶段,有待进一步论证其应用前景。
Objective To continually study the transcatheter closure of secundum atrial septal defeCt (ASD), the ChooSEAL devices were Performed for 4 cases in Pediatric since May 1998.Methods The device include 12. 17.23 .28 .33 .40size. The meommended occluder to scotched defectdiameter is 1. 7~2 .2: 1 according balloon for measurement. RaftS All of four cases were completely occluded, and in which there are tWo holes in oneof case. At moment, no complication, devices replacement, fmpent, interference wilh the strUctUreand residual shunts have been found. The methed handles well, but the application of ASDcase must be less than 20mm diameter. Patient selection for the procedure probably accOUnt for theimprovement of indication. Further clinical trials arenuderway. Advances in detering wether effective ofthe device are being prospectively followed up.[
