32例老年人直肠癌的诊治体会

Diagnosis and management of aged patients with rectal carcinoma report of 32 cases

目的 探讨老年人直肠癌诊治的特殊性。方法与结果 32例老年人直肠癌(>60岁),术前有合并症23例。手术30例,肿瘤切除24例,切除率80％,13例出现并发症,死亡率占6％。结论 1、直肠指检、乙状结肠镜仍是老年直肠癌主要诊断措施;2、重视合并症的诊治及术后监护,是降低术后并发症及死亡率的重要因素。3、手术治疗仍是老年人直肠癌主要治疗方法。应根据不同年龄,肿块距边缘的距离,肿瘤的生物学特征及全身情况采用不同的术式。

objective To investigate the speciality of rectal careinoma diaguosis and managment in aged patients. Methods and Results 30 cases underwent operative treatment among 32 aged patients (>60 years) , other diseases accompanied by in 23 cases, with tumor resection in 24 cases (80%) and complication in 13 cases,3 cases died ( 6 % ) Conclusion Digital rectal examination and reetoromanscopy still are the most important diagnostic methods for aged patients with rectal carcinoma, the diagnosis and treatment of diseases accompanied by and post operation intense care should be laid stress on which are important factors to lower complication of post operation and mortality, according to different age, distanee from tumor to anul edge, biological specific property of tumor and the general situation different operative methods should be taken
