应用DNA指纹对一大城市医院中结核病传染的初步监测:无家可归者和流动工人中暴发流行的检测 被引量:1

摘要 背景:巴黎东南部一所大城市教学医院。目的:通过对1993年3月1日至1994年2月28日确诊的涂阳肺结核患者和1994年3月1日至1995年4月30日确诊的所有结核病患者(任何形式的结核性感染)分离的菌株(161株)进行系统的限定性片断长度多态性分析(RFLP),对结核病医院内传播的初步监测。结果:系统的PFLP分析表明12个患者簇(n=40),这些患者具有相匹配RFLP特征的结核分支杆菌菌株感染。没有耐多药菌株。与非成簇患者相比,成簇患者似乎更多的是无家可归者(55%比19%,P≤0.001)或居住在旅馆的非洲流动工人(20%比6%,P=0.01),并且此前很少住院(12%比28%,P=0.05)。进一步的流行病学调查显示成簇的结核病确非医院内传播所造成,而是由于社会传播,很象是在无家可归者收容所和流动工人的旅店中传播所造成的。结论:在本研究期间的患者中未鉴定出结核病的医院内传播。使用基于医院的样本进行系统的RFLP分析能检出结核病在特定社会环境中的播散。
