摘要
应用电话传输心电系统(TTEM)对125例心脏病患者,其中心律失常患者88例,缺血性心脏病患者37例,在外出旅游途中进行动态监测,并与患者出发前所做的250份心电图(ECG)比较。结果TTEM经过315次呼叫传输,对心律失常的检出率为65.9%,而ECG的检出率为29.5%(P<0.01)。TTEM对心肌缺血的阳性检出率为51.4%,而ECG检出率为18.9%(P<0.01)。为患者提供急救服务24人次,指导用药104人次。提示TTEM是心脏病外出旅游人群监护的及时、可靠、实用的工具。
125 travellers were monitored by TTEM (Transtelephonic Electrocardiograph monitoring). The author Compared 315 TTEM calls with 250 pieces of ECG (Electrocardiogram) before their travelling, the detectable rate of arrhythmia of TTEM was 65.9 %, and that of ECG was 29.5 % (P<0.01). There were 68 persons with short, brief and irregular arrhythmia and 20 persons with continual and repeated arrhythmia among 88 persons with arrhythmia. 55.9% of detectable rate on short, brief and irregular arrhythmia were associated with TTEM as compared with 8.8% of that of ECG (P<0. 001). 100% of detectable rate on continual and repeated arrhythmia were associated with TTEM and ECG. The detectable rate of myocardial ischemia of TTEM was 51.4%, while that of ECG was 18.9% (P<0.01). During the period of monitoring, 104 times of direction to take medicine and 24 times of emergency treatment for travellers were provided, including 14 times in Beijing, 1 time in shanghai, 6 times in Xian, 2 times in Luoyang and 1 time in Taiyuan. If the city to which the travellers has been is one of the members of heart emergency organization, the traveller can the local monitoring center besides calling his center for treatment. The author regards that TTEM has very important value on short, brief and irregular arrhythmia, and has diagnosed value on myocardial ischemia. The author suggests that the following traveller should be monitored by TTEM: (1) Suspected or diagnosed myocardial ischemia. (2) Suspected or diagnosed arrhythmia. (3) After heart surgery or heart involved treatment (PTCA, PBMV, RFCA, implanted cardiac electric pacemaker, etc. (4) Eld people susceptible to coronary heart disease (hypertension, diabetes, hyperlipemia, ect. ). The result of this article shows that TTEM is a reliable, useful and timely tool for travellers with cardiac diseases.
