The Investigation and Analysis of 48 Couples Infected with STD Together

本文报道了1994—1997年6月对泉州地区49,496名各类出入境旅行者监测体检中发现性病感染者251例,对其中93例感染者的配偶进行血清学检查,发现有48人的配偶同时为性病感染者(其中有2例抗-HIV阳性)。对此48对配偶进行流行病学调查,结果表明性乱是导致STD传播的主要途径,提出了加强对入出境旅行者、涉外婚姻、劳务输出人员传染病监测体检,预防和控制STD传播的重要性。

STD has become a global health problem, it spread broadly all over the world and seriouly threaten the social stability and economy development. In order to comprehened the epidemic situation of STD infections in Quanzhou city, survillance examination on the persons entering or leaving Quanzhou city were made from 1994 to June, 1997. A total of 49, 496 persons have been surveyed, 251 cases of STD infections were found, the detectable rate is 0.5%. Among 87 spouses of them, 46 eases were found infected with STD as well, the infected rate was 52.87%. Among 7 HIV infected persons, 3 were foreigners, one was visitor returning from Thailand, and 3 were international seamen, two spouses that concerning foreign marriages were found infected with HIV also. The epidemiological surveys showed that: (a) The STD infection has been increasing year by year, the detectable rate of STD infections is 0. 5% from 1994 to June, n 1997 i Quanzhou, this rate is higher than some other coastal cities. The reason may be that Quanzhou city is a famous hometown of overseas Chinese, and has a large number of persons entering or leaving Quanzhou city every year. (b) Only two spouses of 7 HIV—infected persons were found HIV—seropositive, the others were anti—HIV negative, the reason may be that some of them were still in the incubation period and othcrs had'nt infect with HIV. (c) People may be in fected with STD, but the international seamen, exported workers, self—employed laborers have more chances. (d) Young people aged from 20 to 39 are the high risk group who are susceptible highly to STD, the infection rate in 68.76%. (e) Unsafe sexual behavior was the main way causing STD spread. Among 46 STD infected persons, 32 are male, t4 are female. 31 male and 5 female admitted that had ever experienced unsafe sexual behavior. 7 HIV infected persons had the same experiences also, 1 male denied any sexual behavior, but his wife had ever unsafe induced abortion, 9 female denied any unsafe sexual behavior but their husbands were detected infected with STDs. Most of the infected persons infected with STDs abroad. Between the person and his spouse, most men were usually the source of infections, their wives were infected with STD by sexual contact. (f) Clinical symptoms: the result of physical examination showed only 9 cases suffered from gonorrhea, 5 cases suffered from 'condylomata acuminatum', 3 cases suffered from syphilis mixed with gonorrhoea, lung TB and 'condylomata acuminatum' separately. 70 cases, early incubated syphilis, others have no typical symptoms. The authors Suggested that: (a) According to the materials it has mastered, the STD infection has been increasing year by year, so it is important to enforce the communicable disease surveillance on the persons leaving or entering the territory, especially on the international seamen, the persons concerning transnational marriage and returning from abroad. (b) STD is transmitted mainly through sexual intercourse, especially with prostitutes. In order to reduce the risk of acquiring infection, it is necessary to disseminate the knowledge about preventing STD among the persons leaving or entering the territory, provide some medical consultation, enhance their conciousness of self- protection, make them preserve themselves'purity and take proper measures to protect themselves. (c) With the further opening to the world, the foreigners coming to China are on the rise, the transnational marriage is also on the rise. The health and quarantine organizations must enforce premarital phyiscal examination and communicable diseases surveillance to prevent STD from spreading into China. (d) The incubation period of AIDS is very long and variable, some HIV- seropositive individuals have remained asymptomatic for more than a decade, for this reason, the follow—up survey needs to be applied to the HIV infected persons and their spouses regularly. (e) Since STD is often symptomatic, especially in women, and always mixed with other diseases simultaneously. Most STD infections were found by serological test, persons with possible exposures should consult their physician in time and initiate appropriate measures immediately to prevent STD infection.
