甾体5α还原酶抑制剂与良性前列腺增生药物治疗 被引量：3

摘要 良性前列腺增生(Benign prostatic hy- perplasia,BPH)是老年男性最常见的良性肿瘤,为前列腺尿道周围区细胞良性腺瘤性增生.根据尿流动力学,其症状可分为3类:排尿梗阻、逼尿肌不稳定和逼尿肌收缩力受损.BPH病因主要是前列腺腺体过度生长形成环尿道周围的腺瘤,其次是受肾上腺素能神经调节的膀胱颈或前列腺自身平滑肌肌张力增强而降低了膀胱尿排空能力.前列腺由基质与上皮构成,BPH主要是基质(平滑肌及与之相连结的组织)的增生过程,其平滑肌增生尤为明显,呈现BPH症状的前列腺基质与上皮比率远高于无BPH症状时的比率.BPH为一雄激素依赖性过程,人体雄激素90%由睾丸间质细胞分泌,由肾上腺分泌的雄激素只占10%.研究发现Wolffian管(附睾、输精管、精液囊)的分化成熟依赖于睾酮(testosterone,T),而前列腺及雄性外生殖器的分化成熟有赖于二氢睾酮(dihy-drotestosterone,DHT).
