治疗药物监测方法研究进展

摘要 近年来有关治疗药物监测(TherapeuticDrug Monitoring,TDM)的方法研究报道甚多,一些新技术和方法的推广无疑对临床用药的有效性和安全性发挥了积极作用.本文就临床监护体液中药物浓度检测方法研究进展,且主要就色谱分析技术(HPLC、GC及GC·MS等)的应用分述如下.见诸于文献广泛应用的色谱技术,反相-HPLC法发展.Fraser A.D.等报道于人血清中定量测定抗惊厥新药拉莫三嗪(Lamotrigine)浓度的反相-HPLC法,取血样量仅需200μl,UV检测波长为λ306nm,较常用的【200nm波长处测定的HPLC法远为优越,且较专一.此法适用于治疗药物监察,并可提供临床药动学的研究.另一抗惊厥新药加巴喷丁(Gabapentin)Neurontin)因其分子结构中缺少生色基因,迄今尚无可靠的分析方法以测定血药浓度并用以评价其药动学,Lensmeyer G.L.等选用固相提取分离加巴喷了的方法,旨在改进繁琐衍生化步骤,减少内源性组分的干扰.本法HPLC色谱良好,回收率可达90%以上.
