摘要
本文对近十年来我科收治的59例脑型疟疾的有关临床数据,就预后与意识障碍程度、进入疟区时间、原虫密度、治疗是否及时与贫血程度等进行分析,提示脑型疟预后只与意识障碍程度相关联(P<0.01)。根据我科观察到的脑型疟血液流变学与红细胞免疫功能变化,结合国外近年的研究成果,将恶性疟原虫基因变异等与脑型疟的临床表现通过一系列的病理生理环节串联起来,探讨脑型疟的发病机理。并建议将脑型疟的诊断分为脑型疟前期与脑型疟期两型,此分型有利于预后判断、资料统计及指导临床救治。
It was analysed the relationship between the prognosis of 59 patients with the cerebral malaria admitted by our department in recent 10 years and their dysbulic degree, time entering the malaria epidemic areas, protozoa density, time when treated and anaemic degree. It was found that the prognosis of the patieats was only related to the dysbulic degree(P<0. 01). concerned over the blood dynamies and erythrocytic immunity of the patients , gene variation of the protozoa and the clinical symptoms, it was thought that the developing course of the disease should be furthur defined as two stages-cerebral malaria early stage and cerebral malaria stage, which might be helpful to the diagnosis, prognosis, data statistics and treatment of the disease .
