纤维镜对鼻咽癌鼻腔侵犯的检出及疗效观察

INFLUENCE OF FIBEROPTIC ENDOSCOPY ON THE INVASION OF NASAL CAVITY BY NASOPHARYNGEAL CARCINOMA (NPC) AND ITS PROGNOSTIC VALUE

1989年11月～1991年6月,在利用纤维镜、CT、间接镜对141例鼻咽癌鼻腔受侵的检查中,纤维镜的检出频度(61.7％)高于CT和间接镜,差异有非常显著性;CT的误诊率10.3％,漏诊率31.0％,间接镜的漏诊率54.6％;纤维镜下鼻腔受侵犯可分为游离突入和粘膜浸润二种类型,70Gy/7周后,后者的肿瘤残留率是前者的10倍(3.7％和36.7％)。纤维镜是目前诊断鼻咽癌鼻腔受侵最好的工具。

From 1989 to 1991, invasion of the nasal cavity in 141 NPC patients was examined by the anterior nasal speculum, CT scan and fiberoptic endoscope and the results were compared. The postlive rate of the fiberoptic endoscope was 61. 7% whch was significantly higher than the other two. The erroneous and.the misdiagnosis rates of CT were 16. 7% and 31. 4%. The mis-diagnosis of the anterior nasal speculum was 54. 6%. Under the fiberoptic endoscope, the invasion of the nasal cavity could be classified in to two types; protrusion onto and invasion into the mucosa. After 70 Gy/7 wk, the positive residual rate in the latter was 10 times higher than the.former (36. 7% vs 3. 7%). Fiberoptic endoscopy remains the best means of finding the nasal invasion .by a NPC lesion.
