Two cases of Paneth cell adenoma of the stomach are reported and the histological, ultrastructural, immunohistochemical and immunocytochemical features are described. The surgical specimens showed varied pictures of dysplasia with severe atrophic gastritis and Intestinal metaplasia. 60%-70% of the tumor cells contained coarse cytoplasmic granules stained red with eosin or PAS but remained unstained with aician blue pH2. 5 and exhibited positive immunostaining for lysozyme. The nature of these granules was confirmed by electron microscopy and immunoelectron microscopy. In addition, some enldocrine cells were detected in the neoplastic glands. The characteristics of Paneth cell adenoma of the stomach are discussed. There might be a close relation in the histogenesis with intestinal metaplasia suggested.
