手术创伤后机体激素和代谢变化与不同配方氨基酸代谢支持的影响

摘要 本文通过对45例全胃切除患者进行较全面的研究,发现术后第一天体内分解激素(肾上腺素、去甲肾上腺素和皮质醇)明显高于术前,到第四、八天恢复正常水平。不同配方氨基酸代谢支持对分解激素变化无影响,但个体化氨基酸代谢支持提高了生长激素的含量,与8.5%氨基酸组和10%氨基酸组相比,均有显著性差异(P<0.05),并发现生长激素的变化与24h 尿的尿素氮、3-甲基组氨酸排出量呈显著负相关,与氮平衡呈显著正相关。提示生长激素的增加有利于机体的蛋白质合成代谢,促进机体尽早恢复。 In present study,45 gastric cancer patients having radical gastrectomy were involved to determine traumatic response and effects of metabolic support.The results showed that the catabolic hormons (epinephrine,norepinephrine and cortisone) levels in the 1st postoperative day were significant higher than that in the day before operation,and in the 4th and 8th postoperative days declined to normal range.In individual amino acid (IDAA) group the arised level of human growth hormon (HGH) was significantly different compared to the 8.5% amino acid (AA) group and 10%AA group.It was also found that the level of HGH was negatively correlated to urine urea and 3-MH excretions,and positive to NB.The re.suits indicate that HGH could improve protein synthesis and may be benifit to the patient's early recovery from the trauma.
