摘要
应用免疫组化定量分析法,首次对12例非胰岛素依赖型糖尿病(NIDDM)肾病患者肾活检组织中胰岛素样生长因子-1(IGF-1)受体、表皮生长因子(EGF)和表皮生长因子受体(EGF-R)进行了观察,并结合患者病程、糖尿病肾病分期和肾功能进行了比较。发现NIDDM肾病患者肾小管上EGF和EGF-R的量明显高于正常人。正常人肾小球内EGF染色阴性,4例NIDDM肾病患者肾小球系膜区EGF阳性,且均伴明显系膜细胞和系膜基质增生。正常人和NIDDM肾病患者肾小球内IGF-1受体均为阴性,肾小管上IGF-1受体的量在二者之间也无明显差异。上述结果表明,肾小管上EGF和EGF-R含量增加可能参与NIDDM肾病的发病过程,而肾小球内EGF的出现则提示其与系膜细胞和系膜基质增生有关。
Quantitative analysis of insulin-like growth factor-1 receptor(IGF-1R),epidermal growth factor (EGF)and epidermal growth factor receptor(EGF-R)in renal biopsy specimens were performed in 12 patients with diabetic nephropathy(NIDDM)using immune peroxidase technique(4 layer PAP).It was found that the amount of EGF and EGF-R in renal tubules were markedly inceased as compared with normal control.However,it did not correlate with the course of NIDDM,the stage of diabetic nephropathy or the level of serum creatinine of these patients.EGF was absent in the glomeruli of normal control,but present in glomerular mesangial area of 4 patients with severe proliferation of mesangial cell and the matrix.Unexpectedly,IGF-1R was absent in the glomeruli of diabetic nephropathy as in the normal control.The amount of IGF-1R detected in renal tubules of diabetic nephropathy was not different from normal control.It was suggested that the increased amount of EGF and EGF-R in renal tubules might be responsible for the renal tubular hypertrophy found in NIDDM,while the appearance of EGF in glorneruli could contribute to the abnormal proliferation of mesangial cell and the matrix.
