PPAR-γ2基因Pro12Ala多态性与糖尿病视网膜病变关系的初步研究

Preliminary Study on Relationship Between PPAR-γ2 Gene Pro12Ala Polymorphism and Diabetic Retinopathy

摘要 目的初步探讨PPAR-γ2基因Pro12Ala位点多态性与江西地区汉族人糖尿病视网膜病变(DR)的相关性。方法临床随机筛选出无亲缘关系的研究对象共185例,其中,实验组为根据WHO1999年糖尿病诊断标准和糖尿病视网膜病变诊断标准,诊断为糖尿病视网膜病变的住院和门诊患者66例(男32例,女34例,平均年龄57.94±11.56岁);健康对照组119例(男57例,女62例,平均年龄58.54±12.56岁)。采集研究对象的一般临床特征,应用全自动生化分析仪测定TC、TG、HDL-C、LDL-C、FPG、2hPG等临床指标,放免法测定空腹胰岛素水平,高效液相法测定HbA1c。运用实时荧光PCR方法鉴定研究对象PPAR-γ2基因Pro12Ala基因型;统计学处理运用SPSS11.5统计学软件进行数据统计分析,计数资料间的比较用卡方检验,组间计量资料均以均数±标准差(-x±s)表示,用Logistic回归分析多因素相关性。结果 1.实验组和健康对照组基因型均为PP纯合子,Pro12Ala基因型频率及等位基因频率分布差异均无统计学意义(P>0.05);2.实验组与健康对照组临床资料比较:实验组SBP、FPG、2hPG、HbA1c、FI NS、HOMA-IR均高于健康对照组,HDL-C低于健康对照组,差异有统计学意义(P<0.05);3.Logistic回归分析发现HbA1c为糖尿病视网膜病变发生的独立危险因素。结论 1.PPAR-γ2基因Pro12Ala位点多态性与部分江西地区汉族人2型糖尿病无明显相关性。2.HbA1c为糖尿病视网膜病变发生的独立危险因素。 Objective To preliminarily explore the correlation between Prol2Ala polymorphism in PPAR-γ2 gene and diabetic retinopathy in Han population in Jiangxi. Methods Sixty-six diabetic retinopathy inpatients and outpatients(male cases=32,female cases=34,with the average age of 57.94±11.56) were selected from 185 clinical study objects according to WHO 1999 diagnostic criteria and diabetic retinopathy diagnostic criteria.The control group was composed of healthy check-up outpatients(male cases=57,female cases=62,with the average age of 58.54±12.56).The TG,TC,HDL-C,LDL-C,FPG and 2hPG were detected and analyzed through automatic biochemical analyzer.The level of FINS was detected by radioimmunoassay and HbA1c detected by HPLC.The genotypes of Prol2Ala variant in PPAR-γ2 gene were determined by quantitative real-time polymerase chain reaction.All the results were analyzed with SPSS 11.5 statistic software.The measurement data among groups were analyzed statistically by ±s and the difference of enumeration data was detected by chi-square test.Logistic regression analysis was performed to determine multi-factor correlation. Results Homozygous genotype PP was the only genotype of diabetic retinopathy group and control group,and no statistically significant difference was found in the distribution of genotype frequencies and allele frequencies of Prol2Ala(P0.05).SBP,FPG,2hPG,HbA1c,FINS,and HOMA-IR in diabetic retinopathy group were significantly higher than those in control group,while HDL-C was lower than that of control group,and the difference was statistically significant(P0.05).Logistic regression analysis showed that HbA1c entered Logistic regression equation,and it was the major risk factor for diabetic retinopathy. Conclusions PPAR-γ2 gene Prol2Ala polymorphism is not associated with the incidence of diabetic retinopathy among the Hans in Jiangxi Area.HbA1c is the major risk factor for diabetic retinopathy.
