门控心肌灌注显像对冠心病患者心肌血流灌注及心功能的评价 被引量:2

摘要 目的探讨99mTc-MIBI门控心肌灌注显像(GMPI)评估冠心病患者心肌血流灌注及心功能的价值。方法对30例临床疑似冠心病患者行GMPI,并在行GMPI 1周内行冠状动脉造影(CAG)、超声心动图(UCG)等检查。结果 (1)以CAG为参照,GMPI检测冠心病的灵敏性为88.89%,特异性为83.33%,准确性为86.67%,阳性预测值为88.89%,阴性预测值为83.33%。(2)GMPI检出"罪犯血管"的阳性率左前降支(LAD)为9/10,左回旋支(LCX)为2/3,右冠状动脉(RCA)为5/5。对血管不同病变支数,GMPI检出阳性率单支病变为8/9、双支病变为5/6、三支病变为3/3。(3)GMPI检测的舒张末期容积(EDV)、收缩末期容积(ESV)、左室射血分数(LVEF)、心输出量(CO)与UCG检测结果呈相关性(相关系数r值分别为0.74、0.71、0.63、0.59,P<0.01)。结论 99mTc-MIBI GMPI在冠心病的诊断上具有较高的灵敏性和特异性,对冠心病患者心肌血流灌注及心功能的评价具有重要的临床价值。 Objective To explore the values of gated myocardial perfusion imaging(GMPI) in evaluating the left ventricular function and myocardial perfusion of coronary heart disease patients. Methods Thirty patients with suspected coronary heart disease underwent GMPI examination.Then subsequent coronary angiography,echocardiography and other tests were carried out on these same subjects within one week. Results(1)Comparing with CAG,sensitivity,specificity,accuracy,positive predictive value and negative predictive value of GMPI diagnosis of coronary heart disease were 88.89%,83.33%,86.67%,88.89%and 83.33% respectively.(2)The positive rate of "criminal vessel" detected by GMPI were 9/10 in left anterior descending artery(LAD),2/3 in left circumflex artery(LCX),5/5 in right coronary artery(RCA).The positive rate of single-vessel,two-vessel and multi-vessel stenosis were 8/9,5/6 and 3/3 respectively.(3)Significant correlations existed between the results from GMPI and UCG in determination of end diastolic volume(EDV),end diastolic volume(ESV),left ventricular ejection fraction(LVEF)and cardiac output(CO).Related coefficient were 0.74,0.71,0.63 and 0.59 in EDV,ESV,LVEF and CO respectively(P0.01). Conclusion 99mTc-MIBI gated myocardial perfusion imaging displays high sensitivity and specificity in the diagnosis of coronary heart disease and is of great value in evaluating myocardial perfusion and cardiac function.
