C臂机下经皮肾镜治疗上尿路结石63例

Clinical Evaluation of C-Arm X-Ray Machine in the Guidance of PCNL Treatment of Upper Urinary Calculus

目的:探讨C臂引导下经皮肾镜取石术(PCNL)处理上尿路结石的安全性和疗效。方法:对63例接受C臂引导行PCNL的患者进行回顾分析及总结。结果:经皮肾通道成功建立62例(98.41%),均建立F16-18经皮肾通道并成功碎石,其中单通道56例,双通道5例,三通道1例;47例行Ⅰ期取石,15例行Ⅱ、Ⅲ期取石;结石取净率为88.71%;术中、术后中度明显出血2例,迟发性出血2例。结论:C臂引导下PCNL治疗上尿路结石直观、安全、有效、创伤小,特别适合开展PCNL的早期应用。

Objective： To evaluate the security and efficacy of surgical treatment of upper urinary calculus with percutaneous nephrolithotomy（PCNL） assisted by C-arm X-ray machine.Methods： Sixty-three cases of upper urinary calculus underwent PCNL treatment guided by C-arm X-ray machine were retrospectively analyzed from November 2009 to November 2010.Results： There were 62 patients successfully constructed with renal tract for PCNL（98.41%）,including 47 patients underwent first stage operation,and 15 patients received second or third stage operation.All the percutaneous tracts were established by F16-18 tubes,including 56 single tracts,5 dual tracts,and 3 multi-tracts.The successful rate was up to 88.71%,only two cases had hemorrhage during operation（hemoglobin decreased more than 30% and less than 60%） and two cases had delayed hemorrhage.Conclusion： C-arm X-ray machine could improve the successful rate of percutaneous nephrostomy,especially suitable for the early application of PCNL.
