关节镜下治疗膝关节色素沉着绒毛结节性滑膜炎

Knee Arthroscopy Treatment Experience of Pigmented Villonodular Synovitis 18 Cases

目的探讨关节镜下滑膜切除术治疗膝关节色素沉着绒毛结节性滑膜炎(p igm en ted v illonodu lar synov i-tis,PVN S)的临床疗效。方法对18例膝关节色素沉着绒毛结节性滑膜炎患者在关节镜下进行滑膜切除,汽化烧灼创面,1例配合开放手术,术后追踪观察,评定疗效。结果本组18例均获随访,随访时间6个月～5年,平均2年5个月。根据Lysho lm膝关节评分系统,术前评分为(55.6±4.1)分,术后3个月随访时评分为(87.2±5.7)分。根据徐锦森提出的膝关节功能评定标准,优13例,良4例,差1例,优良率94.4%。结论关节镜下可以准确诊断膝关节色素沉着绒毛结节性滑膜炎,彻底切除关节内病变,必要时配合开放手术,可有效减少术后复发,最大程度地恢复膝关节功能。

Objective To study the clinical curative effection of the knee arthroscopic treatment in Pigmented Villonodular Synovitis,PVNS.Methods 18 cases of knee Pigmented Villonodular Synovitis（PVNS） in patients with arthroscopic for synovial membrane resection,wound vaporization burning,1 case with open surgery,have been observed and evaluated.Results All 18 cases have been followed up,follow-up period from 6 months to five years,the average 2 years 5 months.According to Lysholm knee rating system,the preoperative score is（55.6±4.1） points,and 3 months after operation（87.2±5.7）points.According to knee functional evaluation standard,XuJinSen proposed,optimal 13 cases,good 4 cases,poor 1 case,good rate 94.4%.Conclusion Knee arthroscopic can accurate diagnosis Pigmented Villonodular Synovitis,completely remove intra-articular lesions,when necessary with open surgery,effectively reduce the post-operation relapse,and extremely recover knee function.
