手术与导管溶栓治疗急性下肢深静脉血栓形成的护理

The nursing of thrombectomy and indwelling catheter thrombolysis treatmeat acute deep vein thrombosis of the lower extremity

目的:探讨手术结合置管溶栓治疗急性下肢深静脉血栓形成的护理方法。方法:对138例患者采取手术取栓后在滤器保护下行置管溶栓术,术后经留置导管内滴入溶栓药物,全身应用抗凝治疗,并采取针对性的护理措施。结果:本组患者平均置管5 d。经留置导管静脉造影示:120例患肢深静脉全程通畅,静脉壁光滑无反流,5例腘静脉狭窄50%,10例胫前静脉部分再通,3例胫后静脉部分再通。随访:136例症状全部消失,2例穿弹力袜后能正常工作。结论:手术取栓联合导管溶栓治疗急性下肢深静脉血栓形成疗效肯定,可以减少下肢深静脉血栓形成后遗症。实施合理的护理措施是保证治疗成功的重要环节。

Objective：To explore the nursing of indwelling catheter thrombolysis of acute deep vein thrombosis of the lower extremity after thrombectomy.Methods：138 cases of acute deep vein thrombosis of the lower extremity were indweller catheter thrombolysis after thrombectomy,all belong to mixed thrombus and give pertinency nursing.Results：The average indwelling time was five days.Venography had been done after thrombectomy.There were 120 cases whose deep vein were unblocked,whose wall of deep vein were smooth（no returning to flow）.There were 5 cases whose poplitea vein proved stenosis reaching 50%.10 cases whose tibial anterior vein proved partly unblocked.With visiting：there were 136 cases whose syndrome has dispared totally.2 cases can take normally work by wearing elasticity sock.Conclusion：Indwelling catheter thrombolysis of acute deep vein thrombosis of the lower extremity after thrombectomy is safe and effective,reducing the linger effect of acute deep vein thrombosis of the lower extremity.Reasonable nursing care was a key ring-joint pre and post operation.
