舒芬太尼对大鼠缺血再灌注损伤心肌的保护作用 被引量:2

Protective Effects of Sufentanil on Acute Myocardial Ischemia/reperfusion Injury in Rats

摘要 目的观察舒芬太尼对大鼠急性缺血再灌注损伤心肌(I/R)的保护作用。方法建立72只大鼠心肌缺血再灌注损伤模型,随机分为假手术组(sham组)、缺血再灌注对照组(I-R组)、缺血预处理组(IPC组)和舒芬太尼不同剂量预处理组(SPC1、SPC2、SPC3组),分别以0.25、1、5μg.kg-1静脉泵注5min,停止5min,重复进行3次。于缺血前30min、缺血30min、再灌注90 min时记录左室内压峰值(LVSP),以及左心室收缩及舒张最大压力变化速率(±dp/dtmax)。再灌注90min时取出大鼠心脏,并取结扎线以下的心室壁心肌组织行病理学电镜检查。结果与sham组比较,I-R组、IPC、SPC1、SPC2和SPC3组LVSP及±dp/dtmax降低(P<0.01),心肌超微结构不同程度损伤;与I-R组比较,IPC组、SPC1、SPC2、SPC3组LVSP及±dp/dtmax升高(P<0.01),心肌组织超微结构损伤减轻。结论舒芬太尼可对大鼠急性心肌缺血再灌注损伤心肌产生保护效应。 Objective To explore the protective effects of sufentanil precondition on myocardium ischemia/reperfusion(I/R) injury.Methods The myocardial idchemia and reperfusion injury model was established.The animals were randomly divided into 4 groups: sham operation group(sham,n=12);ischemia-reperfusion control group(I-R,n=12);ischemic preconditioning group(IPC,n=12)and sufentanil preconditioning group which was further divided into 3 subgroups according to the dose of sufentanil(SPC).In I-R group the hearts were subjected to 30 min ischemia followed by 90 min reperfusion(I/R).In IPC group the hearts were subjected to 3 episodes of 5 min ischemia in succession at 5 min interval for reperfusion before I/R.In SPC group the animals received 3 periods of 5 min remifentanil infusion at 0.25(SPCl,n=12),1(SPC2,n=12),5(SPC3,n=12)ug/kg at 5min interval before I/R.LVSP and ±dp/dtmax were recorded before and after I/R.Morphological change of the heart was observed by electron microscopy.Results ①In the ischemic/reperfused period,LVSP and ±dp/dt were significantly lower in the I-R group as compared with the Sham group(P0.01),which were higher in the IPC and SPC groups than those of I-R group(P0.05,P0.01).② Myocardial injury in group IPC and SPC was less severe than that in I-R group.Conclusion Sufentanil precondition has protective effects on acute myocardial infarction against I/R injury in rats.
