不同方式卵巢囊肿保守型手术对卵巢功能影响的临床观察

Clinical observation for the effect of various conservative surgeries on ovarian function

目的分析不同方式卵巢囊肿保守型手术对女性卵巢功能的影响。方法选取2008年2月～2011年1月在我院经腹腔镜卵巢非子宫内膜样囊肿行保守型手术的96例妇女作为研究对象。按照治疗方法的不同,将其随机分为A组(经腹腔镜电凝止血组)、B组(经腹腔镜缝合止血组)和C组(经腹缝合止血组)三组。并对其术前、术后1、6个月的血清FSH(促卵泡素)、LH(促黄体生成素)及E2(雌二醇)进行测定并比较,分析不同方式卵巢囊肿保守型手术对卵巢功能的影响。结果 A组术后1、6个月和术前相比,FSH、LH值均升高,E2值下降,差异性明显,具有统计学意义(P<0.01)。B组术后1、6个月和术前比较,FSH值升高,E2值下降,但差异性不明显,因而不具有统计学意义(P>0.05)。C组术后1、6个月和术前相比较,FSH、LH值均升高,E2值下降,差异性明显,具有统计学意义(P<0.01)。结论应用腹腔镜电凝止血卵巢囊肿剥除术式对女性卵巢功能影响明显,腹腔镜缝合止血卵巢囊肿剥除术式能有效保护卵巢功能,临床上值得进一步推广应用。

Objective To analyze the effect of different ways of conservative surgery for ovarian function. Methods 96 cases of women from February 2008 to January 2011in the hospital treated with laparoscopic ovarian endometrial cyst of non-line conservative surgery and were chosen as research objects.They were randomly divided into group A（laparoscopic coagulation group）,group B（laparoscopic hemostasis group） and group C（to stop bleeding after abdominal suture group） according to different treatments.The serum FSH（follicle stimulating hormone）,LH（luteinizing hormone） and E2（estradiol） preoperative and postoperative 1 month and 6 months were measured and compared.And the impact of different methods of ovarian cysts conservative for surgery on ovarian function was analyzed. Results FSH,LH values were elevated,E2 decreased for A group 1,6 months after treatment compared with preoperative,there were significant differences and the statistical significance（P0.01）.FSH values increased,E2 decreased for Group B 1,6 month after treatment compared with preoperative,but the difference was not significant and there was no statistical significance（P0.05）.FSH,LH values were elevated,E2 decreased for Group C 1,6 month after surgery compared to before,and there was significant differences.There was statistical significance（P0.01）. Conclusions Laparoscopic coagulation type of ovarian cystectomy affect ovarian function of women significantly,laparoscopic ovarian cystectomy hemostasis type can effectively protect the ovarian function,and it is worthy of further clinical application.
