摘要
目的通过比较阻塞性黄疸大鼠行胆肠内引流时切断肝固有动脉与不切断肝固有动脉动物模型的血清胆红素、肝脏病理形态学改变以及肝细胞凋亡率改变,初步探讨阻塞性黄疸大鼠行胆肠内引流时结扎或切断肝固有动脉的安全性及其机制。方法 48只雄性SD大鼠,随机分为A、B、C、D四组。结扎大鼠胆总管造成阻塞性黄疸模型,7d后行胆肠内引流。A、B组不结扎切断肝固有动脉,C、D组结扎切断肝固有动脉。A、C组胆肠内引流后7d处死,B、D组胆肠内引流后14d处死。收集血液和肝脏组织送检测。结果 A组大鼠死亡1只,死亡时间为胆肠内引流后第3d,死亡率8.33%;D组死亡2只,为胆肠内引流后第10、12d,死亡率16.67%。两组死亡率差异无统计学意义(P>0.05)。血清胆红素(TBIL)、凋亡率(AI)C组高于A组,D组高于B组,差异有统计学意义(P<0.05)。肝病理形态学改变(如肝脏组织纤维化、肝细胞坏死及炎细胞浸润)肝固有动脉结扎组均重于非结扎组。结论阻塞性黄疸大鼠行胆肠内引流时结扎切断肝固有动脉不安全,对肝细胞凋亡率有较大影响。
Objective To investigate the safety and mechanism of blocking or cutting hepatic artery in obstructive jaundice rats with biliary enteric drainage,by comparing serum bilirubin,pathological changes and changes of liver cell apotposis in animal models of obstructive jaundice rats cutting or non-cutting hepatic artery during biliary enteric drainage.Method 48 male SD rats were randomly divided into groups A,B,C and D,and made into obstructive jaundice model by ligating common bile duct,enteric drainage was adopted and hepatic artery was cutted in groups C and D 7 days later.Rats in groups A and C were scarificed 7days later while rats in groups B and D were killed 14 days later.Speciments of blood and liver tissue were collected to test.Results One rabbit died in group A,the time of death was the third day after biliary enteric drainage,the rate of death was 8.33%.Two rabbits died in group D,the time of death was the 10th and 12th day after biliary enteric drainage,the rate of death was 16.67%.The difference of death rate between two groups had no statistical significance(P0.05).The TBIL and AI in group A were higher than those in group C,while those in group D were also higher than those in group B,and the difference had statistical significance(P0.05).The morphological changes of liver pathology(such as liver fibrosis,liver cell necrosis and inflammatory cell infiltration) in hepatic artery ligation group were more serious than those in non-ligation group.Conclusions It is unsafe to cut hepatic artery,and it has a great impact on apoptosis of liver cells when biliary drainage was underwent in obstructive jaundice rats.
