二尖瓣口舒张早期血流速度峰值与二尖瓣环舒张早期运动速度之比对舒张性心力衰竭的诊断价值 被引量:6

摘要 目的研究二尖瓣口舒张早期血流速度(E)与二尖瓣环舒张早期运动速度(Em)的比值(E/Em)对舒张性心力衰竭的诊断价值。资料与方法回顾性分析2006-07~2009-12临床诊断的舒张性心力衰竭44例以及同期健康对照75例,分析其组织多普勒(TDI)检测的超声心动图特点,并采用ROC曲线分析检测结果。结果 E/Em诊断舒张性心力衰竭的受试者工作特征曲线下面积达到0.882(95%CI:0.802~0.962)。取E/Em>9.3为切点,其诊断舒张性心力衰竭的敏感性和特异性分别为84.1%和82.7%。结论 E/Em对舒张性心力衰竭具有较高的诊断价值。 Purpose To investigate the diagnostic value of the ratio (E/Em) of early diastolic transmitral velocity (E) to early diastolic tissue velocity (Em) measured by tissue Doppler imaging in diastolic heart failure. Materials and Methods Forty-four patients clinically diagnosed as diastolic heart failure and 75 healthy controls from July 2006 to December 2009 were analyzed in this retrospective study. Clinical manifestations and echocardiographic characteristics were analyzed. ROC curve was used to analyze the diagnostic value of E/Em in diastolic heart failure. Results The area under curve of E/Em was 0.882(95%CI: 0.802~0.962) .When the cut-off value of E/Em was defined as above 9.3, the sensitivity and specificity were 84.1% and 82.7% respectively for diagnosing diastolic heart failure. Conclusion E/Em can be used to diagnose diastolic heart failure with high accuracy.
