Application of Susceptibility Weighted Imaging(SWI) in Diagnosis of Neonatal Subependymal/Intraventricular,Intraparenchymal,Intracerebral Hemorrhage

摘要 目的探讨新生儿室管膜下-脑室内出血(SEH/IVH)、脑内出血在磁敏感加权成像(SWI)上的表现。资料与方法搜集新生儿重症监护病房的97例新生儿并同时行头颅MR常规序列(T1WI、T2WI)和SWI序列的扫描。结果 97例新生儿SWI共检出SEH/IVH13例,大脑实质内出血(IPH)7例,小脑内出血(ICEH)10例。常规MR序列检出SEH/IVH8例,IPH3例,ICEH5例。SEH/IVH、IPH、ICEH在SWI上表现为多发斑点状、类圆形、不规则形或铸形的低信号,类球形出血灶在相位图上表现为X-Y轴平面中间层面中心为低信号,外周为高信号,Z轴前后两极层面为低信号。结论较常规MR序列,SWI能更敏感地显示SEH/IVH、IPH、ICEH,表现为典型顺磁性物质的信号特征。 Objective To explore the SWI appearance of subependymal /intraventricular hemorrhage(SEH/IVH),intraparenchymal hemorrhage(IPH),intracerebellar hemorrhage(ICEH).Materials and Methods 97 cases of neonate obtained from neonatal intensive care unit received conventional MRI(T1WI、T2WI) and SWI.Results SEH/IVH,IPH and ICEH in 13,7 and 10 patients were depicted by SWI respectively.8,3 and 5 cases were only found on conventional MR images respectively.In SWI image,the lesions had mottling,oval shape,irregular or casting shape of hypointensity.In phase image,the sphere like hemorrhage was presented with centre low signal surrounded by high signal ring in equator plane and low signal in two poles of sphere along Z axis of main magnetic field.Conclusion Compared with conventional MRI,SWI is more sensitive for detecting SEH/IVH,IPH and ICEH with appearance of typical paramagnetic materials.
