苏北地区消化系统疾病患者的心理社会因素研究

Survey on Social and Psychological Factors of Patients with Digestive Diseases in Northern Area of Jiangsu

目的探讨苏北地区消化系统疾病患者的心理社会因素及其与疾病的相关性,评价患者的心理状态及其相关因素,为消化系统疾病的临床治疗提供新的视角。方法采用生活满意度指数B量表(LSIB)、抑郁自评量表(SDS)和社会支持评定量表(SSRS)对112例消化系统疾病患者(患病组)和108例健康者(对照组)进行问卷调查,资料采用t检验和2χ检验相关分析。结果患病组的生活满意度指数低于对照组,差异有统计学意义(P<0.01);抑郁指数高于对照组,差异有统计学意义(P<0.01);社会支持总分和三个维度的得分均低于对照组,差异有统计学意义(P<0.01)。患者的生活满意度与社会支持中的社会支持总分、主观支持、客观支持呈正相关(P<0.01),与抑郁指数呈负相关(P<0.01);患者的抑郁指数和社会支持度中的客观支持和社会支持总分呈负相关(P<0.05)。患病组的抑郁发生率高于对照组,差异有统计学意义(P<0.05)。结论不同心理状态和社会支持对消化系统疾病的影响不同;消化系统疾病患者的心理社会问题较突出,应有针对性地对消化系统疾病患者提供心理支持和社会支持,改善患者的生活质量和疾病的预后。

Objective To investigate the social and psychological factors of patients with digestive diseases and the factors associated with diseases,and to assess the psychological state and its related factors,so as to provide a new perspective for clinical treatment of digestive diseases.Methods The Life Satisfaction Index B Scale（LSIB）,self-rating depression scale（SDS） and the Social Support Rating Scale（SSRS） were taken for conducting the questionnaire survey among 112 patients with digestive diseases and 108 normal subjects.The data were analyzed by using t test and correlation analysis.Results The life satisfaction index of patients with digestive diseases was significantly lower than that of normal people（P0.01）;depression index was significantly higher than that of the normal（P0.01）;the total score of social support and the standard scores of three dimensions were lower than those of the normal（P0.01）.The life satisfaction of the patients was significantly positively correlated with the total score of social support,scores of subjective and objective support in social support factors（P0.01）,significantly negatively correlated with the depression index（P0.01）;and the depression index was significantly negatively correlated with score of objective support and total score of social support（P0.05）.The prevalence of depression in patients with digestive system diseases was significantly higher than that of normal people（P0.05）.Conclusion Different mental states and social supports have different impacts on the digestive system diseases.The patients with digestive diseases have prominent psycho-social problems.It is important to give necessary psychological support and social support to the patients with digestive diseases so as to improve their quality of life and prognosis of disease.
