摘要
目的通过Mata分析综合评价产前糖皮质激素治疗对早产儿神经系统发育的影响。方法通过计算机检索中国知网、维普、万方、PUBMED、EBSCO和Cochrane图书馆从1980年1月至2010年12月国内外公开发表的有关产前糖皮质激素治疗影响早产儿神经系统发育的有关文献。按Cochrane系统评价方法,采用revman5.0进行Meta分析。结果共入选18项研究,9 955例患者,Meta分析结果显示,对于预防早产儿脑室周围低密度灶(PVL)、脑室内出血(IVH)、严重脑室内出血(Severe IVH),不同糖皮质激素剂型、多疗程与单疗程的治疗效果差异无统计学意义。地塞米松组减少IVH及Severe IVH发生优于未使用产前糖皮质激素组,而倍他米松组减少IVH发生优于未使用产前糖皮质激素组。结论产前使用糖皮质激素治疗可以减少早产儿IVH及Severe IVH的发生。不同糖皮质激素剂型、不同疗程产前糖皮质激素对早产儿PVL、IVH及Severe IVH的影响无明显差异。
Objective To evaluate the effect of antenatal glucocorticoids treatment for the development of the premature infant nervous system.Methods A search for relevant articles published between 1990 to 2010 was run on CNKI,WeiPu,WanFang,Pubmed,EBSCO and Cochrane library database.All researches on the treatment of premature infants with antenatal glucocorticoids were included.The revman 5.0 software was used for Meta-analysis according to Cochrane system evaluation method.Results 9 955 patients were included in 18 studies.Meta-analysis showed that there was no statistical significance to decrease the periventricular leukomalacia(PVL),intraventricular hemorrhage(IVH) and severe intraventricular hemorrhage(Severe IVH) of premature infants in the difference of treatment results between different types,single and multiple courses of antenatal glucocorticoids.Comparing Dexamethasone with no antenatal glucocorticoids,Dexamethasone was better than no treatment in IVH and Severe IVH;Comparing Betamethasone with no antenatal glucocorticids,Betamethasone was better than no treatment in IVH.Conclusion Antenatal glucocorticoids can decrease the occurence of IVH and Severe IVH in premature infant.For different types and different courses of antenatal glucocorticiods,there is no significant difference in the nervous system of premature infant neurodevelopment between PVL,IVH and Severe IVH.
