适用创伤骨科外周血细胞复检规则的探讨

The Discussion of Hematology Review Criteria for Traumatic Orthopedics

目的制定适用创伤骨科的外周全血细胞复检规则。方法采用Sysmex XE-2100全自动血细胞分析仪检测骨科临床标本共计1050份,同时制备双份血涂片进行双盲镜检。参考"国际血液学41条复检规则"、"XE-2100复检协作组"23条复检规则,初步制定适用创伤骨科的复检规则,用840例标本对初步制定的复检规则及6个改进方案进行评估。选用210份临床标本对复检规则进行验证。结果使用初步制定的复检规则假阳性率19.05%、假阴性率2.5%、复检率为22.02%。改进后最终复检规则假阳性率10.36%、假阴性率2.86%、复检率为12.98%。根据统计结果并结合临床医师的意见确定该方案为适用创伤骨科的外周全血细胞复检规则。经验证试验,满足假阴性小于5%的要求。结论使用本规则复检率明显减低,提高了工作效率,适用于创伤骨科。

Objective To establish a suitable hematology review criteria for traumatic orthopedics.Methods The total 1050 clinical blood samples are detected by Sysmex XE-2100 hematology analyzer and microscopic examination.Base on ＂41 Items of International Consensus Review Rules＂ and the 23 Hematology Review criteria established by ＂XE-2100 review criteria organization＂,we established the initial hematology review criteria.Using 840 test data and microscopic examination results,we evaluated the initial criteria and the six improved options.210 samples were used to verify the improved options.Results Apply to the initial criteria,the false positive ratio was 19.05%,the false negative ratio was 2.5%,and the recheck ratio was 22.02%.While apply to the final criteria,the false positive ratio was 10.36%,the false negative ratio was 2.86%,and the recheck ratio was 12.98%.According to the statistical result and the clinical suggestion,the final hematology review criteria were confirmed for traumatic orthopedics.The result of verification test showed the false negative ratio within 5%.Conclusion Using the criteria reduced the recheck ratio,improved the work efficiency,they were suitable for traumatic orthopedics.
