摘要 机械通气是危重病患者的重要救治技术之一。已经明确撤机失败根本原因是呼吸肌力与呼吸负荷的不平衡。重视机械通气与心脏功能的相互作用,预防获得性肌无力,尤其是与机械通气相关的肌无力,控制原发病,对患者其他脏器及气道保护能力等全身情况评估和治疗,可以降低脱机失败和再插管率。新的通气模式及监测的开创,机械通气有创、无创的应用,认识机械通气治疗的复杂性,正确评价临床效果及其局限性。总之,合适的脱机方法,适时动态个体化脱机,能缩短机械通气时间,减少其并发症,提高脱机的成功率。
