黏蛋白(MUC2)在不同胃疾病中的动态表达及其意义

Mucin(MUC2)expression in gastric diseases and its clinical significance

目的:研究黏蛋白(MUC2)在不同胃疾病组织中的表达,以揭示其参与胃癌发生、发展过程中的规律。方法:以免疫组化的方法对243例胃疾病病人进行MUC2蛋白原位表达的检测,并采用结肠直肠癌(CRC)及其癌旁组织作为对照。胃疾病组包括浅表性胃炎组(SG)87例,萎缩性胃炎组(AG)87例,胃癌组(GC)69例,其中包括高中分化腺癌(HMDA)27例、低分化腺癌(PDA)30例、黏液腺癌(MA)或印戒细胞癌(SRCC)12例;结肠直肠癌组80例,包括HMDA 30例、PDA 26例、MA或SRCC 24例。结果:黏蛋白MUC2在AG组较SG组及GC组的表达显著增高(P<0.05);MUC2在胃低分化腺癌中普遍低表达或不表达,其阳性率(40%)较黏液腺癌或印戒细胞癌(75%)显著降低(P=0.040)。结论:黏蛋白MUC2可以作为萎缩性胃炎的分子标志物。MUC2可作为鉴别低分化腺癌与黏液腺癌或印戒细胞癌的免疫组化辅助诊断指标。

Objective The MUC2 protein expression in different gastric diseases was investigated,in order to reveal the role of mucin（MUC2） in the development of gastric cancer.Methods Using immunohistochemical methods,the MUC2 expression was detected in 243 cases of different gastric diseases so was also in cases of colorectal cancer（CRC） accompanied by their adjacent tissues as controls.The gastric diseases were composed of superficial gastritis（SG） 87 cases,atrophic gastritis（AG） 87 cases,and gastric cancer（GC） 69 cases which included the high-moderately differentiated adenocarcinoma（HMDA） 27 cases,poorly differentiated adenocarcinoma（PDA） 30 cases,mucinous adenocarcinoma（MA） or signet ring cell carcinoma（SRCC） 12 cases.Eighty cases of colorectal cancer and their adjacent tissues group,included HMDA 30 cases,PDA 26 cases,MA and SRCC 24 cases.Results MUC2 expression in the AG group was significantly higher than that in the SG and GC groups（P0.05）;and MUC2 expression in the gastric PDA group was generally low or even with no expression,the positive rate（40%） was significantly lower than that in the MA or SRCC group（75%,P= 0.040）.Conclusions The MUC2 can be used as a molecular marker in GA;MUC2 might to be a diagnostic indicator for the identification of PDA and MA/SRCC.
