周围体外循环辅助下右侧乳房下小切口微创左房室瓣手术临床分析 被引量:1

摘要 目的总结50例右侧乳房下小切口微创行左房室瓣手术的临床经验和疗效。方法采用右侧乳房下小切口结合头灯光源辅助方法共对50例左房室瓣疾病患者实施体外循环左房室瓣置换或成形手术(微创组),所有患者采用单腔气管插管,仰卧位,右侧垫高20~30°,右上肢悬吊,插股动脉供血管、双极股静脉引流管的插管技术实施周围体外循环。手术主要切口为右侧乳房下小切口6~8cm(经该切口第4肋间进胸可进行心脏操作)+两孔(右侧锁骨中线第2肋间放置长冷灌针头和牵引上腔静脉阻断带、右侧腋中线第4肋间放置特制的升主动脉阻断钳和牵引下腔静脉阻断带),术者佩戴头灯,经切口可提供良好的视野光源。经房间隔切口或房间沟切口进行左房室瓣手术。对照组为同期传统正中开胸左房室瓣手术的患者50例。结果微创组无中转开胸、延期拔除气管插管、重要脏器功能衰竭、血红蛋白尿及手术死亡病例,4例术中低潮高频通气出现低氧血症,处理后好转,1例术后因肋间肌肉血管出血再次原切口进胸止血。微创组与对照组相比,手术时间、体外循环时间、主动脉阻断时间、体外辅助时间、术后拔除气管插管时间差异无统计学意义;而微创组术后住院时间缩短,术后胸液引流量、住院期间输血量明显减少(P<0.05)。出院前复查心脏超声示左房室瓣瓣膜位置、活动良好,无瓣周漏,左房室瓣成形术后微量或轻度反流,右房室瓣轻度反流,成形效果满意。结论右侧乳房下小切口第4肋进胸结合术者头灯光源辅助、周围体外循环的方法行微创左房室瓣手术,手术安全,术野暴露良好、操作简便易掌握,较常规正中开胸手术创伤小、出血少、住院时间短、切口美观隐蔽,术后早期效果理想。
