摘要
目的:研究环氧合酶-2(COX-2)-1195GA多态性与2型糖尿病对牙周炎易感性的影响。方法:采用病例对照实验设计,选择46例单纯2型糖尿病患者、76例糖尿病伴牙周炎患者、158例单纯牙周炎患者以及112例健康对照者。应用聚合酶链反应—限制性内切酶片段长度多态性基因分析方法(PCR-RFLP),采用卡方检验比较COX-2基因-1195GA位点的基因型和等位基因频率在各组间的分布差异,并计算其与牙周炎的危险度(OR)。结果:与健康组比较,COX-2基因-1195GA位点的AA基因型以及等位基因A在牙周炎组和糖尿病伴牙周炎组中存在统计学差异(P=0.053和P=0.017);2型糖尿病对慢性牙周炎的危险度为1.667(95%CI 0.658~4.222,P=0.281),AA+GA基因型对慢性牙周炎的危险度为2.027(95%CI 1.135~3.621,P=0.017),两者叠加效应对慢性牙周炎的危险度为2.130(95%CI 1.104~4.108,P=0.024),差异具有统计学意义。结论:COX-2基因的-1195GA多态性可能提高了2型糖尿病患者对慢性牙周炎的易感性。
Objective: To explore the risk of COX-2-1195GA polymorphism in type 2 diabetes with chronic periodontitis.Methods: A case-controled study was conducted in 4 groups of patients:diabetes(n=46),diabetes with periodontitis(n=76),periodontitis(n=158) and healthy controls(n=112).Genotypes were determined by polymerase chain reaction restriction fragment length polymorphism(PCR-RFLP).The distributions of genotypes were compared by χ2 test,and the odd ratios were calculated.Results: The A allele of-1195GA had a higher distribution in the group of diabetes with periodontitis than in the controls(P=0.053 and P=0.017).the OR was higher in patients with AA+GA genopyte combined with diabetes(OR=2.130,95%CI 1.104-4.108,P=0.024) than in those with the AA+GA genotype(OR=2.027,95%CI 1.135-3.621,P=0.017) or diabetes(OR=1.667,95%CI 0.658-4.222,P=0.281) alone.Conclusion: COX-2-1195GA polymorphism and type 2 diabetes might have a synergistical effect on the risk of chronic periodontitis.
