多种方法联合应用在桂西壮族妇女宫颈病变筛查中的价值

Value of joint application of multiple methods in screening of cervical lesions in women of Zhuang nationality in western Guangxi

目的:探讨多种方法联合应用在桂西壮族妇女宫颈病变筛查中的价值。方法:选取同时行宫颈液基薄层细胞学检查(TCT)、高危型人乳头状瘤病毒(HR-HPV)检测、细胞DNA定量分析和活组织病理学检查的430例患者的临床资料进行分析。结果:以活检病理结果作为诊断的"金标准",TCT、HR-HPV检测和二者联合检测的检出率分别为64.2%、58.5%和94.3%。TCT与HR-HPV检测的检出率比较,差异无统计学意义(P>0.05)。联合检测与单独TCT或单独HR-HPV检测的检出率比较,差异均有统计学意义(P<0.01);联合检测与TCT联合DNA定量分析的检出率比较,与DNA定量分析联合HR-HPV检测的检出率比较,差异均无统计学意义(P>0.05)。结论:宫颈脱落细胞的TCT联合HR-HPV检测是广西壮族自治区宫颈病变筛查的最佳方案。

Objective：To explore the value of joint application of multiple methods in screening of cervical lesions in women of Zhuang nationality in western Guangxi.Methods：The clinical data of 430 cases who received Thinprep cytology test（TCT）,high risk human papillomavirus（HR-HPV） detection,DNA quantitative analysis,biopsy and pathological examination were selected for analysis.Results：The result of pathological examination after biopsy was used as gold standard of diagnosis,the detection rates of TCT,HR-HPV detection,combined detection of TCT and HR-HPV were 64.2%,58.5% and 94.3%,respectively.There was no significant difference in detection rate between TCT and HR-HPV detection（P0.05）.There was significant difference between combined detection of TCT and HR-HPV and single detection（TCT or HR-HPV detection）（P0.01）.There was no significant difference in detection rate between combined detection of TCT and HR-HPV and TCT combined with DNA quantitative analysis or DNA quantitative analysis combined with HR-HPV detection（P0.05）.Conclusion：TCT of cervical exfoliative cells combined with HR-HPV detection is the best scheme for screening of cervical lesions in Guangxi province.
