重度子痫前期并发低蛋白血症与妊娠结局的相关性

Correlation between severe preeclampsia combined with hypoproteinemia and pregnancy outcome

目的:探讨重度子痫前期患者并发低蛋白血症与妊娠结局的相关性。方法:选择宁波市明州医院收治的重度子痫前期患者80例,依据空腹血清白蛋白结果分为非低蛋白血症组13例、轻度低蛋白血症组26例和重度低蛋白血症组41例。记录各组终止妊娠方式、终止妊娠时间、新生儿出生体重、新生儿窒息、围生儿死亡情况以及孕妇的各项并发症。结果:①随着低蛋白血症程度加重,孕妇终止妊娠时间缩短,剖宫产和治疗性引产病例及孕妇并发症发生率明显增加,自然分娩率明显下降,各组比较有统计学差异(P<0.05)。②随着低蛋白血症程度加重,新生儿出生体重下降,围生儿死亡率明显增加,三组之间比较差异均有统计学意义(P<0.01),但三组新生儿窒息发生率比较差异无统计学意义(P>0.05)。结论:重度子痫前期合并低蛋白血症,易导致孕妇及围生儿不良妊娠结局;血清白蛋白可以作为监测重度子痫前期病情严重程度的常规临床观察指标。

Objective：To explore the correlation between severe preeclampsia combined with hypoproteinemia and pregnancy outcome.Methods：80 cases with severe preeclampsia were selected from the hospital,then they were divided into non-hypoproteinemia group（13 cases）,mild hypoproteinemia group（26 cases） and severe hypoproteinemia group（41 cases） according to fasting serum albumin levels.The ways to terminate pregnancy,the times of pregnancy termination,neonatal birth weights,the incidences of neonatal asphyxia,mortalities of perinatal infants and incidences of various maternal complications in the three groups were recorded.Results：With the aggravation of degrees of hypoproteinemia,the times of pregnancy termination shortened,the numbers of cases choosing cesarean section and therapeutic induced abortion and the incidences of maternal complications increased obviously,the natural delivery rates decreased significantly,there was significant difference among the three groups（P0.05）.With the aggravation of degrees of hypoproteinemia,the neonatal birth weights decreased obviously,the mortalities of perinatal infants increased significantly,there was significant difference among the three groups（P0.01）,but there was no significant difference in the incidence of neonatal asphyxia among the three groups（P0.05）.Conclusion：The pregnant women with severe preeclampsia combined with hypoproteinemia are prone to adverse pregnancy outcomes of pregnant women and perinatal infants;serum albumin can be used as a routine index to monitor the degree of severe preeclampsia.
