206例强直性脊柱炎患者综合工作能力研究

Study on comprehensive work ability indicators in 206 cases of ankylosing spondylitis

目的探讨强直性脊柱炎(AS)患者疾病活动性与综合工作能力(comprehensive work ability,CWA)的关系。方法采用问卷法对强直性脊柱炎患者(AS组)206例及正常对照组206例的CWA进行测评,同时计算观察对象的Bath强直脊柱炎疾病活动指数(BASDAI)、Bath强直性脊柱炎功能指数(BASFI)、强直性脊柱炎整体指数(BAS-G),并填写焦虑自评量表(SAS)及抑郁自评量表(SDS)。结果①AS组CWA指数(comprehensive workability index,CWAI)明显低于正常对照组,32.5±8.6vs 55.1±6.9(P<0.01);②AS组SAS和SDS值超过正常对照组(66.1±9.3)分vs(45.4±7)分(P<0.05);(61.8±11.4)分vs(41.9±8.3)分(P<0.05);③AS疾病活动指数(BASDAI,BASFI,BAS-AG)和CWAI呈负相关(r=-0.326、-0.407、-0.371,P<0.05);④AS组的抑郁状态与焦虑状态(SDS值和SAS值)和CWAI呈负相关(r=-0.296、-0.249,均P<0.05)。结论 AS患者的CWA下降,CWA下降与疾病活动、焦虑及抑郁相关。

Objective To understand the change in the comprehensive work ability index（CWAI） in patients with ankylosing spondylitis（AS）, and its relationship with AS indexes, and anxiety or depression Scale. Methods Comprehensive work ability was assessed in 206 AS patients and compared with that in health controls using the CWAI form, Bath AS Disease Activity Index（BASDAI）, Bath Functional Index（BASFI）, Bath AS Global Score（BAS-G）, Zung Self-Rating Anxiety Seale（SAS）. Zung Self-Rating Depression Scale（SDS） was evaluated at sametime. The results of AS patients were compared with those of healthy controls,the relationship between CWAI and several AS activity index and SAS/SDS was further studied by regression analysis. Results （2）AS patients showed significantly lower CWAI than those of healthy controls,32.5＋8.6 vs 55.1±6.9（ P 〈0.01）. （1）Higher SAS value and SDS value in AS patients were observed as compared with those of healthy controls,（66.1±9.3） scores vs （45.4±7.1） scores（ P 〈0.05） ;（61.8± 11.4） scores vs （41.9 ± 8.3） scores（ P 〈0.05）. （3）The disease activity indexes （BASDAI, BASFI, BAS-AG） were negative correlated with CWAI （ r = -0. 326, -0. 407,-0. 371, P 〈 0.05）. （4） The depression state and the anxiety state（SDS and SAS values） in AS patients were negative correlated with CWAI （ r = - 0. 296, - 0. 249, all P 〈0.05）. Conclusion AS patients demonstrated a remarkable loss of CWAI, and loss of CWAI is associated with disease activity,anxiety and depression.
