宫颈环形电切术治疗164例宫颈疾病的疗效分析

Loop electrosurgical excision procedure in the treatment of cervical leisions in 164 cases

目的探讨宫颈环形电切术（LEEP）治疗宫颈疾病的疗效。方法回顾性分析新野县人民医院2008年5月至2011年4月门诊和住院患者经阴道镜活检初步诊断为宫颈病变者164例,所有患者均行LEEP术治疗,记录术中手术时间,术中出血量,术后病理结果,术后宫颈塑形情况。结果 164例患者中宫颈上皮内瘤变（CIN）68例,平均手术时间为7.2（3~16）min,术中出血量平均为10.2（5~34 ml）,治愈率为95.6%（65/68）,术后宫颈塑形满意率为89.4%（59/68）;宫颈良性疾病96例,其中宫颈尖锐湿疣17例,宫颈息肉22例,宫颈肌瘤8例,中重度宫颈糜烂49例。该组平均手术时间为6.8（2.0~14.0）min,术中出血量平均为9.4（3.0~22.0）ml,治愈率为94.9%（91/96）,术后宫颈塑形满意率为88.9%（85/96）;CIN组与宫颈良性疾病组在平均手术时间、术中出血量、宫颈塑形满意率及治愈率等方面比较差异均无统计学意义（P〉0.05）。结论 LEEP术治疗CIN和宫颈良性疾病简单、安全行之有效。

Objective To evaluate the effect of loop electrosurgical excision procedure（LEEP）in the treatment of cervical leisions.Methods One hundred and sixty-four cases of cervical lesions were analysed retrospectively from May 2008 to August 2011 in Xinye people＇s hospital.The patients were initial diagnoed by the colposcopy and then underwent LEEP surgery.The intraoperative surgical time,blood loss,postoperative pathological findings and postoperative cervical regeneration were evaluated.Results Sixty-eight patients were diagnosed as cervical intraepithelial neoplasia（CIN） with a mean operative time was 7.2（3-16） min,average blood loss was 10.2（5-34）ml,the cure rate was 95.6%（65/68）,and cervical regeneration was 89.4%（59/68）;96 cases of cervical benign disease,of which 17 cases of cervical warts,cervical polyps in 22 cases,8 cases of cervical fibroids,in 49 cases of severe cervical erosion.The mean operative time was 6.8（2.0-14.0） min,average blood loss was 9.4（3.0-22.0） ml,the cure rate was 94.9%（91/96）,postoperative cervical regeneration was 88.9%（85/96）;There was no obvious statistical difference between CIN and cervical benign disease in the average operation time,blood loss,cervical regeneration or the cure rate（P0.05）.Conclusion LEEP treatment of CIN and cervical benign leisions is simple,safe and effective.
