摘要
目的探讨低分子肝素钙治疗肺心病并呼吸衰竭的临床疗效。方法回顾性分析2009年4月~2010年12月肺心病急性发作并呼吸衰竭患者32例,按治疗方法分为两组,对照组采用综合治疗,包括抗感染、改善通气和换气,治疗组在对照组的基础上,行腹壁皮下注射低分子肝素钙80IU/Kg,每天2次,12小时一次,1个疗程为7d。结果治疗后,两组的pH变化差别无统计学意义,但PaCO2比对照组降低,而PaO2明显升高,差别有统计学意义,治疗组中,有8例显效,有效6例,2例无效,总有效率为87.5%;对照组中,显效6例,有效5例,无效5例,总有效率为68.8%;两组差异有统计学意义(P<0.05)。结论低分子肝素钙对改善肺循环,改善呼吸功能,纠正缺氧,值得推广。
Objective To research the low molecular weight heparin treatment of pulmonary heart disease and respiratory failure clinical efficacy. Methods A retrospective analysis of hospital from April 2009 to December 2010 acute respiratory failure disease onset and 32 cases of patients, randomly divided into two groups, the control group was used by the combined treatment, including antibiotics, improved ventilation and ventilation, treatment group were based on the re-abdominal subcutaneous low molecular weight heparin 80IU/Kg, twice a day, once every 12 hours, a treatment for 7d. Results After treatment, the change of the pH value of the two groups difference was not statistically significant, but PaCOz lower than the control group, while the PaO2 significantly increased, the difference was statistically significant; the treatment group, markedly in 8 cases, effective in 6 cases, ineffective in 2 cases, the total effective rate was 87.5%; the control group, markedly in 6 cases, effective in 5 cases, ineffective in 5 cases, the total effective rate was 68.8%. The treatment group was better than the control group in Clinical manifestations (P〈0.05). Conclusion Low molecular weight heparin with the pulmonary circulation and respiratory function had improved, hypoxia corrected, improved myocardial metabolism, myocardial contractility can be enhanced.
