游离膝降动脉股内侧穿支皮瓣修复下肢软组织缺损

Transplanting the free descending genicular artery pedis perforator flap for repairing the limb soft-tissue defect

目的 探讨应用游离膝降动脉股内侧穿支皮瓣修复下肢远端软组织缺损的方法和效果.方法 2009年1月-2010年4月,应用游离膝降动脉股内侧穿支皮瓣修复四肢软组织缺损8例,皮瓣面积：5cm×8cm～6cm×15cm,5例以膝降动脉主干为血管蒂,3例以膝降动脉股内侧穿支为血管蒂.结果 8例皮瓣全部存活,术后随访6～18个月,皮瓣外观及质地良好,感觉恢复S3级.结论 膝降动脉股内侧穿支皮瓣血管解剖恒定,皮瓣厚薄适中,操作简便,是修复四肢远端皮肤缺损有用的办法.

Objective To study a novel method to repair skin defect on distant limbs by applying the flap pedicled with descending genicular artery and evaluate the clinical outcomes. Methods From January 2009 to April 2010, 8 patients with skin defect on the distant limb were treated. The area of the flap were 5cm×8cm-6cm×15cm, 5 cases pedicled with the descending genicular artery and the else 3 cases pedicled with the perforator of the descending genicular artery. Results The mean follow-up was range 6 to 18 months. All the flaps survived. The appearance and texture of the flaps were good. All the flaps＇sensory recovery obtained S3. Conclusion Free descending genicular artery pedis perforator flap is an effective method for treatment of soft tissue defect in distant limbs, owing to its invariable anatomy of vessel pedicle comparable thickness of the flaps, simple procedure and time saving.
